1. සම්පූර්ණ කරන ලද අයදුම්පත (MTA 2 පෝරමය)
2. ජාතික හැඳුනුම්පත
3. අයදුම්කරුගේ ඡායාරූප 02ක් (ග්‍රාම නිලධාරි / සමාදාන විනිසුරු සහතික කළ)
4. යතුරුපැදියේ පැති පෙනුම හා ඉදිරි පෙනුම දිස්වන අයුරින් චැසි අංකය හා එන්ජින් අංකය සහිත ඡායාරූප 02 ක්
5. ආනයන ලියකියවිලි
6. මිළදී ගැනීමේ කුවිතාන්සිය
7. ලියාපදිංචි කිරීමේ ගාස්තුව

2010 අගෝස්තු මස 31 දිනට පෙර මිලදී ගත් 50cc අඩු යතුරු පැදි සඳහා ආනයනික ලියකියවිලි සහ මිලදී ගැනීමේ කුවිතාන්සිය වෙනුවට පහත ලියකියවිලි ඉදිරිපත් කළ හැක.

1. යතුරුපැදිය කෙදිනක සිට අයදුම්කරු සතුව පවතින්නේද යන වග ප්‍රකාශිත දිවුරුම් ප්‍රකාශයක්.
2. (යතුරු පැදිය කිසිදු බැඳුම්කරයකට යටත් නොවන බවට, යතුරු පැදිය පිළිබඳ සියලු වගකීම් අයිතිකරු විසින් භාරගන්නා බවත් එහි අඩංගු විය යුතුය).
3. යතුරුපැදිය තමා සන්තකයේ පවතින බවට ප්‍රදේශයේ ග්‍රාම නිලධාරී ගෙන් ලබා ගත් සහතිකයක්.
4. ලියාපදිංචි කිරීමට ප්‍රථම යතුරුපැදියේ චැසි අංකය හා එන්ජින් අංකය නිවැරැදි බවට මෝටර් රථ පරීක්ෂකවරයෙකු මඟින් MTA 2 අයදුම්පත සහතික කළ යුතුය.

Registration of Motorcycles (Mopeds) below 50CC - Vanathavilluwa Divisional Secretariat

What to bring

1. Completed Application (MTA Form 2)
2. National Identity Card
3. 02 photographs of the applicant (Attested by Village Officer / Justice of the Peace)
4. 02 photographs showing the side view and front view of the motorcycle with chassis number and engine number
5. Import documents
6. Purchase receipt
7. Registration fee

For motorcycles below 50cc purchased before 31st August 2010 the following documents can be submitted in lieu of import documents and purchase receipt.

1. An affidavit stating since when the motorcycle is in possession of the applicant.
2. (It should contain that the motorcycle is not subject to any bond and that the owner assumes all responsibility for the motorcycle).
3. A certificate obtained from the local village official that the motorcycle is in his possession.
4. The MTA 2 application must be certified by a motor vehicle inspector that the chassis number and engine number of the motorcycle are correct before registration.
